First things first, open the App Store, then download an app called, Chameleon - Widget Wallpapers.

Now, open the app and tick mark the places according to your current home screen setup. Let's add a medium size widget and some apps. Once done, save it.

Now open the Photos app and set the saved image as a wallpaper on your iPhone.

That's it. The medium sized widget I used in this setup is from Chameleon app, it's a free widget, you can add it from widgets area.
